Contact forms should be simple — you just need to get the information that you need in order to get back in contact with the person who submitted the form. We recommend having fields for the person’s name, email and phone number. If your form is complicated, people are less likely to fill it out.
Some fields aren’t necessary, such as the “Where did you hear about us?” field. You can find this information from your Analytics, and it’s not really necessary to have in your form. If you find yourself questioning whether something is necessary to include, it probably isn’t.
Make sure that your contact form is easy to find — you don’t want people to have to search for it if they want to get in touch with you.
